Purpose The purpose of this field memorandum is to provide specific guidelines that are unique to this facility in order to fully implement standard operating procedure312.02.01.001. The warden or designee has oversight. General Statement This field memorandum is subject to revision at the discretion of the warden of this facility, chief of the Operations Division (or designee), or director of the Idaho Department of Correction. Completion of Worksheets2Reference2 CONTACT INFORMATIONIf there are suspicious circumstances surrounding the death, the scene is to be secured and the appropriate law enforcement agency will be requested to respond and investigate. The Ada County Sheriff’s Department telephone number is 377-6790.The coroner is responsible for establishing a cause of death when circumstances suggest that death did not occur as a result of natural causes.The coroner is responsible for transport of the body out of the institution. The coroner may delegate this responsibility to an associate. The telephone number for the Ada County Coroner is 287-5556.The coroner will coordinate the transfer of the body to the Boise Funeral Home, 8209 Fairview Ave, Boise, ID 83704, phone # 322-2998, unless other arrangements have been made by the family and at their expense. COMPLETION OF WORKSHEETSThe shift commander will complete Appendix A of SOP 312.02.01.001 Death of an Offender and submit it to the warden’s office along with supporting documentation (e.g. 105 Incident Notification Report).Notification of the next of kin will be handled by the warden’s office (Appendix B of SOP 312.02.01.001). The warden’s management assistant will oversee the completion of Appendix C of SOP 312.02.01.001 Death of an Offender. This form will be used to track date and time of contacts made.The Warden’s AdministrativeAssistant will have oversight of the delivery of the death packet, per instructions contained in SOP 312.02.01.001 Death of an Offender.
